Tenuate is used along with a doctor-approved, reduced-calorie diet, exercise, and behavior change program to help you lose weight. It is used in people who are significantly overweight (obese) and have not been able to lose enough weight with diet and exercise alone. Losing weight and keeping it off can reduce the many health risks that come with obesity, including heart disease, diabetes, high blood pressure, and a shorter life.

It is not known how this medication helps people to lose weight. It may work by decreasing your appetite, increasing the amount of energy used by your body, or by affecting certain parts of the brain. This medication is an appetite suppressant and belongs to a class of drugs called sympathomimetic amines.

TENUATE (diethylpropion or amfepramone) is a stimulant similar to an amphetamine. Diethylpropion is an appetite suppressant that affects the central nervous system.

Tenuate is used as together with diet and exercise to treat obesity in people who have a body mass index (BMI) of at least 30 kilograms per square meter.

Tenuate is usually given after diet and exercise have been tried without success.

You should not take Tenuate if you are in an agitated state, or if you have pulmonary hypertension, severe coronary artery disease, overactive thyroid, glaucoma, severe high blood pressure, or a history of drug abuse.

Do not use this medicine if you have used a MAO inhibitorin the past 14 days, such as isocarboxazid, linezolid, methylene blue injection, phenelzine, rasagiline, selegiline, or tranylcypromine.

Do not take Tenuate together with any other diet pills unless your doctor tells you to.

Tenuate may cause blurred vision and may impair your thinking or reactions. Diethylpropion may also cause restless feelings that can hide the symptoms of extreme tiredness. Be careful if you drive or do anything that requires you to be alert and able to see clearly.

Tenuate may be habit-forming and should be used only by the person it was prescribed for. Keep this medicine in a secure place where others cannot get to it.

Do not stop using Tenuate suddenly after long-term use, or you could have unpleasant withdrawal symptoms. Ask your doctor how to avoid withdrawal symptoms when you stop using this medicine.

Take the immediate-release form of this medication by mouth, usually 3 times a day 1 hour before meals or as directed by your doctor. If late-night eating is a problem, your doctor may direct you to take another dose in the evening. Taking this medication late in the day may cause trouble sleeping (insomnia).

The extended-release form of diethylpropion is usually taken once a day in the mid-morning. Do not crush or chew extended-release tablets. Doing so can release all of the drug at once, increasing the risk of side effects. Also, do not split the tablets unless they have a score line and your doctor or pharmacist tells you to do so. Swallow the whole or split tablet without crushing or chewing.

The dosage is based on your medical condition and response to therapy. Your doctor will adjust the dose to find the best dose for you. Use this medication regularly and exactly as prescribed in order to get the most benefit from it. To help you remember, take it at the same time(s) each day.

Diethylpropion is usually taken for only a few weeks at a time. It should not be taken with other appetite suppressants (see also Drug Interaction section). The possibility of serious side effects increases with longer use of this medication and use of this drug along with certain other diet drugs.

This medication may cause withdrawal reactions, especially if it has been used regularly for a long time or in high doses. In such cases, withdrawal symptoms (such as depression, severe tiredness) may occur if you suddenly stop using this medication. To prevent withdrawal reactions, your doctor may reduce your dose gradually. Consult your doctor or pharmacist for more details, and report any withdrawal reactions right away.

Though it helps many people, this medication may sometimes cause addiction. This risk may be higher if you have a substance use disorder (such as overuse of or addiction to drugs/alcohol). Do not increase your dose, take it more often, or use it for a longer time than prescribed. Properly stop the medication when so directed.

You should see some weight loss soon after you start this medication. Tell your doctor if you do not see at least 4 pounds of weight loss within 4 weeks of starting this medication. This medication may stop working well after you have been taking it for a while. Talk with your doctor if this medication stops working well. Do not increase the dose unless directed by your doctor. Your doctor may direct you to stop taking this medication.

BEFORE TAKING THIS MEDICATION:

Do not use Tenuate if you have used an MAO inhibitor in the past 14 days. A dangerous drug interaction could occur. MAO inhibitors include isocarboxazid, linezolid, methylene blue injection, phenelzine, rasagiline, selegiline, tranylcypromine, and others.

You should not use Tenuate if you are allergic to diethylpropion, or if you have:

  • pulmonary hypertension;

  • severe coronary artery disease;

  • severe high blood pressure;

  • an overactive thyroid;

  • a history of drug abuse;

  • glaucoma; or

  • if you are in an agitated state.

Do not take Tenuate with any other diet pills unless your doctor tells you to. Taking diethylpropion in combination with other diet pills or appetite suppressants can cause a rare fatal lung disorder called pulmonary hypertension.

To make sure Tenuate is safe for you, tell your doctor if you have ever had:

  • kidney disease;

  • high blood pressure;

  • a heart valve disorder or heart rhythm disorder;

  • seizures or epilepsy; or

  • if you have taken any other diet pills within the past 12 months.

It is not known whether this medicine will harm an unborn baby. If you use Tenuate while you are pregnant, your baby could become dependent on the drug. This can cause withdrawal symptoms in the baby after it is born. Tell your doctor if you are pregnant or plan to become pregnant.

Diethylpropion can pass into breast milk and may affect the nursing baby. Tell your doctor if you are breast-feeding.

Tenuate is not approved for use by anyone younger than 16 years old.



SIDE EFFECTS

Dizziness, dry mouth, difficulty sleeping, irritability, nausea, vomiting, diarrhea, or constipation may occur. If these effects persist or worsen, notify your doctor or pharmacist promptly.

Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.

This medication may raise your blood pressure. Check your blood pressure regularly and tell your doctor if the results are high.

Tell your doctor right away if any of these unlikely but serious side effects occur: fast/irregular/pounding heartbeat, mental/mood changes (e.g., agitation, uncontrolled anger, hallucinations, nervousness), uncontrolled muscle movements, change in sexual ability/interest.

Stop taking this medication and seek immediate medical attention if any of these rare but very serious side effects occur: severe headache, slurred speech, seizure, weakness on one side of the body, vision changes (e.g., blurred vision).

This drug may rarely cause serious (sometimes fatal) lung or heart problems (pulmonary hypertension, heart valve problems). The risk increases with longer use of this medication and use of this drug along with other appetite-suppressant drugs/herbal products. If you notice any of the following unlikely but very serious side effects, stop taking this medication and consult your doctor or pharmacist right away: chest pain, difficulty breathing with exercise, decreased ability to exercise, fainting, swelling of the legs/ankles/feet.

A very serious allergic reaction to this drug is rare. However, seek immediate medical attention if you notice any of the following symptoms of a serious allergic reaction: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.

For dosage forms and strengths that allow once-a-day dosing, taking the dose about 10 to 14 hours before bedtime 

For dosage forms and strengths that require more than one dose per day to be taken, taking the last dose of the day at least 4 to 6 hours before bedtime
For extended-release dosage forms—Swallowing whole; not breaking, crushing, or chewing

IMMEDIATE-RELEASE: 25 mg orally 3 times a day, 1 hour before meals, and in mid-evening if desired to overcome night hunger.

CONTROLLED-RELEASE: 75 mg orally once a day in mid-morning, swallowed whole.

Comments:
-Indicated for monotherapy use only.
-Treatment should be continued only if the patient has satisfactory weight loss within the first 4 weeks of treatment (e.g., weight loss of at least 4 pounds or as determined by the physician and patient).
-Discontinue treatment if tolerance develops; do not exceed the recommended dose in an attempt to increase the effect.

Use: Management of exogenous obesity as a short-term adjunct (a few weeks) on a regimen of weight reduction based on caloric restriction in patients with an initial BMI of 30 kg/m2 or higher and who have not responded to appropriate weight reducing regimen (diet and/or exercise) alone.
